Central Family Life Center is proud to be part of the Staten Island Social Care Network (SCN), a program connecting Medicaid members across Staten Island and select neighborhoods in Brooklyn and Queens to free services that support health and well-being.

Central Family Life Center’s Social Care Network (SCN) services are now expanding beyond Staten Island to support Medicaid members in select neighborhoods in Brooklyn and Queens.

Eligible members in the following zip codes may now access SCN screening and navigation services:

Brooklyn: 11209, 11219, 11220, 11228
Queens: 11377, 11373, 11368, 11355

All eligible participants can receive free support for housing, food, transportation, job assistance, and benefits navigation through our Social Care Navigators.
